FCH Vision:

Residents of rural northwestern CT and the greater Harlem Valley of New York have access to high quality health-related services that contribute to their overall health and well-being.  The Foundation for Community Health (FCH) is a leader and catalyst for the development of innovative and effective rural health delivery systems that focus on prevention, access and well-developed community-level collaborations.  FCH sets the standard for regional, rural philanthropies.

FCH Mission:

The Foundation for Community Health is a private, not-for-profit philanthropic organization formed to maintain and improve the physical and mental health of the residents of the area historically served by Sharon Hospital with an emphasis on serving those most vulnerable.

To accomplish this mission, we:

  • Work together with local healthcare and social service providers;
  • Encourage innovation and evidence-based, effective practices;
  • Focus on prevention, access and collaboration efforts;
  • Collaborate with other foundations and with government;
  • Advocate policy changes that improve health in rural areas; and
  • Adhere to the highest standards of ethics and accountability.

Specifically, we:

  • Fund grants for specific programs and services;
  • Provide technical and program development grants and assistance to increase effectiveness and capacity of local services and programs;
  • Convene stakeholders to work on community issues;
  • Promote collaborations;
  • Evaluate existing health care services;
  • Make Program-related investments;
  • Conduct Research;
  • Support Advocacy.
