How to Apply

The Foundation for Community Health (FCH) awards grants to organizations and institutions that directly impact the Foundation's mission of improving the health status of people in its service area. FCH Grantmaking can be categorized into three types:

Solicited and Unsolicited Grants

  1. Solicited grants are those offered by direct invitation or via a competitive Request for Proposals (RFP) process.  These grants will directly address one of our priority areas under our Strategic Grantmaking funding.
  2. Unsolicited grants may or may not directly fall within the Foundation’s strategic grantmaking priority areas but still must have a positive impact on the health and well-being of our communities.  Letters of Intent for unsolicited grants are reviewed as received (Opportunistic Grantmaking for more details), however potential applicants are encouraged to call to discuss their ideas in advance with FCH program staff.

Two step process for all requests for funding

Submit a Letter of Intent.  If it is approved, submit a Full Proposal.

  1. Letter of Intent (includes):
    1. A two page letter describing the organization, the proposed project, what it hopes to achieve, and how you will know it is successful.
    2. Organizational Coversheet
    3. Budget Worksheet
  2. Full Proposal (includes):
    1. Organizational Coversheet
    2. Written Narrative (no more than 10 pages)
    3. Budget Worksheet and Justification Form (See page 2 of the Budget Worksheet)
    4. Logic Model Form (provided)
    5. Required Supporting Documents include:
      1. Up to three Letters of Support, Partnership or Collaboration
      2. Copy of Agency (or if a very large agency the appropriate Department) Budget
      3. Board and staff rosters relevant to this program
      4. Annual Report (if available)
      5. IRS Letter 501 (c) (3) Determination Letter
      6. Most recently completed Form 990
      7. Most recent audit or review
