1. What is the purpose of The Foundation for Community Health (FCH)?
The Foundation for Community Health (FCH) is a private, not-for-profit foundation dedicated to improving the health and well being of the residents of the greater Harlem Valley in New York and the northern Litchfield Hills of Connecticut with an emphasis on serving those most vulnerable.

3. How was the Foundation for Community Health (FCH) formed?
The Foundation for Community Health (FCH) was established in 2003, when Sharon Hospital was sold to for-profit Essent Healthcare, Inc. Connecticut law requires that the proceeds from the sale of a not-for-profit hospital to a for-profit organization remain in the charitable sector through the creation of a “conversion” foundation (such as the FCH). The original settlement was for approximately $16 million. The process was guided by Connecticut’s Attorney General and formalized by a decision of the Superior Court of the State of Connecticut.

5. What relationship does the Foundation for Community Health (FCH) have with Berkshire Taconic Community Foundation (BTCF)?
The Foundation for Community Health (FCH) is organized as a “functionally integrated” supporting organization (“Type III”) of BTCF. BTCF nominates three persons to the Foundation for Community Health’s Board of Directors. BTCF also acts in an advisory capacity to the Foundation for Community Health on programmatic and administrative matters. In turn, the Foundation for Community Health provides BTCF with information and assessments of health-related issues for BTCF’s health-related grantmaking. The Foundation for Community Health also contracts with BTCF for financial, administrative and investment services.
6. What relationship does the Foundation for Community Health (FCH) have with Sharon Hospital ?
The Foundation for Community Health (FCH) works cooperatively with the for-profit Sharon Hospital, currently owned by Essent Healthcare, Inc., to ensure and enhance a network of affordable and accessible health care services in the region. The Foundation for Community Health (FCH) cannot fund programs operated by or for the benefit of any for-profit institution, including Sharon Hospital.

8. What type of activities does the Foundation for Community Health (FCH) fund?
The Foundation for Community Health (FCH) provides funding within three primary areas: Mental Health, Oral Health, and Access to Health Services. In addition, there is limited funding under the title of Opportunity Grants for proposals outside the 3 priority areas. These proposals are developed in consultation with the Foundation for Community Health (FCH) staff.
9. What is a grant proposal and why is one necessary?
The Foundation for Community Health’s (FCH) funding process involves an applicant submitting a written grant proposal. It is necessary because the proposal identifies and describes: who is requesting funding, the reason why funding is being requested, the details of the project to be funded, the project’s expected outcomes, the process for evaluating the project’s outcomes, and a budget for the proposal.
Possibly. All non-profit organizations are strongly encouraged to call the Foundation for Community Health (FCH) staff to discuss ideas for programs and services that seek to improve the health of our constituents. This may include strengthening the organization delivering the services.
