How to Apply
The Foundation for Community Health awards grants to organizations and institutions that directly respond to its current priority areas: Mental Health, Oral Health and Access to Services. The Foundation also accepts discretionary grant proposals from time to time that are responsive to the general foundation’s mission of improving the health status of people in its service area.
Solicited and Unsolicited Grants.
- Solicited grants are those offered by direct invitation or via a competitive Request for Proposals (RFP) process. These grants will directly address one of our priority areas and often have deadlines for submission.
- Unsolicited grants may or may not directly impact the Foundation’s priority areas but must still intend to have a positive impact on the health and well being of our communities. Letters of Intent for unsolicited grants are reviewed as received but potential applicants are encouraged to call to discuss their ideas in advance with Foundation program staff.
Two step process for all requests for funding.
First submit a Letter of Intent, if approved, submit a Full Proposal.
- Letter of Intent (includes):
- 2 page letter describing the organization, the proposed project, what it hopes to achieve, and how you will know it is successful.
- Organizational Coversheet
- Budget Worksheet
- Full Proposal (includes):
- Organizational Coversheet
- Written Narrative (no more than 10 pages)
- Budget Worksheet & Justification Form (See page 2 of the Budget Worksheet)
- Logic Model Form (provided)
- Required Supporting Documents include:
- Up to three Letters of Support, Partnership or Collaboration
- Copy of Agency or if a very large agency the appropriate Department Budget
- Board and staff rosters relevant to this program
- Annual Report (if available)
- IRS Letter 501 (c) (3) Determination Letter
- Most recently completed Form 990
- Most recent audit or review
