Application Forms

The Foundation for Community Health (FCH) has a two step process for all requests for funding. First, submit a Letter of Intent. Then, if approved, submit a Full Proposal.

For your convenience, we have provided downloadable, fill–in PDF or Microsoft Word™ forms. Each form can be downloaded individually. For the entire package, select Complete Set of Proposal Forms (PDF only). *

There are three options for using the fill–in forms:

1. Download and save a copy of the forms to your hard drive to work offline (Note: this option is available only if you have the complete Adobe Acrobat™ Application ).
2. Open the forms using Adobe Acrobat Reader™ to fill in the forms online.
3. If you would like to work with the forms in Microsoft Word™, they are available for download (below the PDF versions). Automatic calculations are unavailable for these versions.
