Strategic Plan 2005 - 2007
Goal
To improve the mental health of the residents of our communities by improving access to quality community-based comprehensive behavioral health services.
Strategies
- To strengthen communication, cross-training and coordination between providers of community-based mental health and substance abuse services by bringing together behavioral health providers to:
- identify strengths, as well as gaps, in current services;
- explore areas for strengthening and building capacity of current providers;
- identify opportunities for collaborating and streamlining activities;
- identify professional training and educational needs emphasizing the intersection of substance abuse and mental health;
- learn about best practices regarding community-based prevention and early intervention services.
- To promote mental health and substance abuse prevention and early intervention services by offering funding opportunities to those working with at-risk populations
- To increase community awareness of behavioral health by supporting community communication campaigns.

To improve access to quality healthcare services in our communities.
FCH Goals & Strategies
- To improve capacity of, and coordination between, community-based emergency and non-emergency health-related transportation services by convening providers and users of services to identify strengths as well as gaps in services and by providing funding opportunities which enhance and increase services.
- To improve access to quality health related care, specifically for vulnerable populations such as youth, elderly, poor and uninsured by providing funding opportunities.
- To develop FCH as a local resource on rural health access issues.
Goal
To improve oral health in our communities by improving access to quality community-based comprehensive services and by increasing awareness about the importance of oral health to our overall health and wellbeing.
Strategies
- To quantify the prevalence of dental disease by documenting evidence of disease found in 3rd graders in the service area;
- To support prevention efforts that promote good oral health and which emphasize its connection to nutritional issues like obesity, heart disease and diabetes by offering funding opportunities;
- To collaborate with others (providers, other foundations, government officials, and consumers) in influencing state budgets and decisions which encourage and develop more access points for dental care by participating in regional and statewide efforts and educating appropriate governmental leaders, while ensuring that rural issues are addressed.
The Foundation has a small amount of funds available to respond to an issue or request outside FCH Program Priority Areas in a reasonable amount of time. Examples of an “opportunity” might include:
- a response to an emergency or unexpected event,
- a unique idea or pilot project, or
- a request by another area funding partner to join them in a collaborative effort to improve the health of our communities.
Donor Restricted grants are those gifts received by the Foundation with specific restrictions on the use of the funds. The Foundation for Community Health will continue its fiduciary role in honoring and carrying out the wishes of those funds donated by individuals, families, or organizations, etc. for specific health-related purposes. Currently the Foundation operates 3 such funds:
Prescription Assistance Fund — The Foundation will continue to direct and support its Prescription Assistance Program which originated from the directives of several donor advised funds to help pay medical expenses of those in need.
Sue Stanton Memorial Fund — This fund is restricted to funding in the area of pediatric health.
Medical Education Fund — The Foundation is working with the medical community to identify and support learning opportunities for health care providers on community health issues.
